1、申领、填写有关表格未注册的单位首先根据 《注册程序》 申请注册登记，已在贸促会注册登记的申请单位，应由指定的原产地证申领员持贸促会颁发的《原产地证明书申领员证》向所在地的贸促会申领由贸促会统一印制的《一般原产地证明书申请书》和《中华人民共和国出口货物原产地证明书》，并准确、真实填写其中各栏。
1. Unregistered units shall first apply for registration in accordance with the Registration Procedures. Applicants who have registered with CCPIT, The designated certificate-of-origin applicant shall apply for general Certificate of Origin application and Certificate of Origin for Export goods of the People's Republic of China uniformly printed by the council for the promotion of International Trade (CCPIT) with the certificate of Origin Application issued by ccPIT and fill in each column accurately and truthfully.
2、提交原产地申请申请单位出口每批货物时，应于货物报关出运前三天向签证机构申请办理原产地证，并严格按照签证机构的要求，真实、完整、正确地填写下列材料：When exporting each batch of goods, the applicant shall apply for a certificate of origin to the visa authority three days before the customs declaration and shipment, and fill in the following materials truthfully, completely and correctly in strict accordance with the requirements of the visa authority:
《一般原产地证明书/加工装配证明书申请书》填制申请书时，HS编码取前六位，FOB总值和拟出运日期要准确无误，要加盖申请单位印章，并由申领人签名；
When filling out the Application form for General Certificate of Origin/Certificate of Processing and Assembly, the HS code shall be the first six digits, the TOTAL FOB value and the intended shipment date shall be accurate, and the seal of the applicant shall be affixed and signed by the applicant.
《中华人民共和国出口货物原产地证明书》一式四份；
出口货物的商业发票一份；
签证机构认为必要的其他证明文件。
Certificate of Origin of Export Goods of the People's Republic of China in quadruplicate;
One copy of commercial invoice for export goods;
Other supporting documents deemed necessary by the visa authority.
